Job Title: Certified Application Counselor (CAC) / Enrollment Specialist
Reports to: Deputy Director
Program: Medicaid Expansion/MassHealth Members Outreach & Enrollment (CCC Network, in partnership with Health Care for All)
Location: 264 Griffin Street, Fall River, MA
Status: Part time 25-27 hours per week, Grant-Funded (one-year term; possible renewal)

Position Overview:
Citizens for Citizens, Inc. (CFC) seeks a full-time Certified Application Counselor (CAC) / Enrollment Specialist for the statewide Connecting Communities to Care (CCC) network, coordinated by Health Care For All (HCFA). This role provides culturally responsive outreach, education, and enrollment assistance to the Fall River Community in navigating MassHealth and Health Connector coverage — helping them understand eligibility changes, respond to notices, use the MyServices portal, and apply for, renew, or regain coverage. Priority populations include Medicaid Expansion members ages 19–64, people experiencing homelessness, immigrants and refugees, older adults, people with disabilities, and residents who speak languages other than English. CAC certification is completed through HCFA/MassHealth training after hire, with ongoing support from the statewide network.
Principal Duties and Responsibilities (Essential Functions):
Duties include but not limited to:
● Conduct community-based outreach on MassHealth changes, redetermination timelines, and available supports.
● Educate residents on MassHealth/Health Connector eligibility, notices, deadlines, and required documentation.
● Guide residents in using the MyServices portal and share reliable vaccine information.
● Build relationships with local partners (health centers, shelters, schools, faith and immigrant/refugee-serving organizations).
● Complete CAC certification training and maintain certification in good standing.
● Provide one-on-one help applying for, renewing, or regaining MassHealth and Health Connector coverage.
● Resolve coverage gaps, lapses, or denials; maintain accurate, confidential case records.
● Screen for and refer residents to Medicare Savings Programs, food assistance, My Ombudsman, and CFC’s internal programs (housing, food, childcare, energy assistance).
● Participate in HCFA’s CCC network trainings and technical assistance; track outreach/enrollment data and submit required reports.
Qualifications & Skills Required:
1. High school diploma or equivalent (Associate’s/Bachelor’s preferred) or equivalent experience.
2. Willingness to complete CAC certification within first 30–60 days of employment.
3. Strong interpersonal skills; ability to build trust across diverse communities.
4. Ability to learn public benefits eligibility rules and systems; basic computer proficiency.
5. Reliable transportation for local outreach travel.
6. Commitment to person-first, non-stigmatizing approaches to working with people navigating poverty or health system barriers.
Preferred (not required):
● Bilingual/multilingual (e.g., Portuguese, Spanish).
● Prior experience with MassHealth, Health Connector, or public benefits enrollment.
● Experience with older adults, people with disabilities, people experiencing homelessness, or immigrant/refugee communities.
● Familiarity with Fall River-area community resources.
Working Conditions:
Combination of office-based work and community outreach (health centers, shelters, events, home visits as appropriate). Occasional evenings/weekends for outreach events; some local travel required.
Physical Requirements:
While performing the duties of this position, the employee may occasionally be required to perform minimal physical duties such as standing, walking, sitting for lengthy periods, and reaching with hands and arms, and stooping. The role requires sitting for periods of time and extensive use of the telephone and computer for case documentation. The employee will regularly travel locally to conduct outreach and may occasionally be required to physically lift up to 25 pounds for files, outreach materials, or event supplies.

Citizens for Citizens, Inc. Head Start/ Early Head Start
Preschool Teacher Assistant: Full Day, Full Year
Responsibilities include but are not limited to:
● In the absence of the Teacher assumes full responsibility of the classroom or center as well as the supervision of the substitute and any volunteers.
● Assists the Teacher with planning activities and learning experience of the children.
● Participates in parent/guardian conferences, home visits (2 per year minimum) and team conferences as part of the interdisciplinary team.
● Assists the Teacher with evaluating and assessing individual children to determine needs and individual programming.
● Assist Teacher with completing necessary paperwork.
● Assist The Teacher in developing parent activities at the center or classroom level and encourages parents to participate in the program.
● Complies with all Head Start Performance Standards and Department of Early Education & Care Regulations.
● In addition to above, any reasonable task deemed necessary by the project Director to meet goals and objectives.

Job Title: Nurse
Department: Head Start
Reports to: Health & Nutrition Coordinator
Non-Exempt
Position Overview:
● Maintain updated health information on children assigned.
● Maintain the health and safety regulations for the centers assigned.
Principal Duties and Responsibilities (Essential Functions):
● Participate in the health screening and intake process for all children in/or entering the program.
● Participate in planning, assessing and identifying the medical needs of the program. Integrate health education into the classrooms and other program activities.
● Participate as a member of the interdisciplinary team at center conferences and attend required staff meetings and training sessions.
● Submit all required reports and control sheets to the Health & Nutrition Coordinator and prepare individual health plans on children when necessary.
● Give First Aid if a child is ill or injured when qualified staff is not available.
● Advise parents of their child’s progress regarding weight and height.
● Participate in the preparations, implementations and recording of information from physical examinations, hearing, vision and dental screenings, and TB tests for children, staff members and volunteers.
● Review children’s medical and immunization records before entering into the program and help parents complete such records.
● Transfer pertinent information to school health records before the child enters the public school system if requested.
● Participate in a disability evaluation meeting with the public school personnel.
● Input medical information when received from the parent into the computer system.
In addition to above, any reasonable task deemed necessary by the project Director to meet goals and objectives.
Qualifications & Skills:
An attitude which is consistent with the nursing profession and is consistent with principles of early childhood education and Head Start Performance Standards. A graduate of an approved school of nursing who is currently registered to practice nursing in Massachusetts. Ability to initiate and maintain good interpersonal relationships with children, parents and other staff members.
Must have a valid driver’s license and reliable vehicle.

Job Title: Family Advocate
Union Position
Responsibilities include but are not limited to:
● Act as Team Leader, facilitating interdisciplinary team meetings and managing children’s records.
● Develop a Parent, Family, and Community Engagement Plan with each family.
● Plan and implement family and community activities with parent and staff input.
● Ensure monthly parent meetings; elect and support Policy Council representatives.
● Organize individual and group activities for parents.
● Conduct home visits to complete paperwork and build relationships with Head Start & Early Head Start families.
● Make referrals to social, mental health, and educational agencies; track outcomes.
● Document all family and team communications accurately in the program database.
● Support recruitment efforts and enrollment.

Employee Benefits
The following summary of employee benefits is provided for informational purposes. Additional information can be provided by the CFC’s Human Resources Office.
Medical
Citizens for Citizens, Inc. (CFC) shall make Health Insurance available to all eligible employees. CFC contributes no more than seventy-five percent of the health insurance premium. The remaining twenty-five percent shall be paid by the employee through payroll deductions. Part-time employees entitled to participate in the Health Insurance plan pay on a pro-rated basis.
Dental
CFC shall make Dental Insurance available to all eligible permanent employees. The agency pays fifty percent of the dental insurance premium; the remaining fifty percent shall be paid by the employee through payroll deductions. Permanent part-time employees with a regularly scheduled workweek of twenty-five hours are eligible to participate.
Life Insurance
CFC provides a life insurance to all eligible permanent full-time employees equal to one time their annual salary plus $10,000 up to the maximum amount of $110,000. The entire premium of the life insurance is paid by CFC.
Disability Insurance
CFC shall provide a disability insurance plan to all eligible permanent employees. The entire premium of the disability insurance shall be paid by the Agency.
Tax Sheltered Annuity (Retirement) Plan
CFC offers a 403 (b) tax deferred annuity for all eligible employees wishing to participate. Employees can contribute up to the annual amount allowable under IRS regulations. At present CFC provides a matching contribution of 5% of the employees’ annual wages if the employee contributes 2% of their annual wages to the plan.
Paid Time Off
Paid time off includes 11 holidays per year and 3 personal days.
Sick Time
For permanent full time employees sick leave accrues at the rate of 1.5 days per month up to a maximum of 65 days (455) hours. Sick leave for part time employees regularly schedules to work 25 hours is prorated. CFC complies with the Massachusetts sick leave law for all eligible employees.
Annual Leave
Permanent full-time employees receive five days of annual leave for every six months of continuous employment. Employees with less than two years of continuous service receive two weeks annual leave. Employees with more than two years and less than five years continuous service receive three weeks annual leave. Employees with more than five years of continuous service receive four weeks of annual leave. Employees with twenty years of continuous service receive five weeks annual leave. Permanent part-time employees whose regularly scheduled work week is twenty-five hours or more shall be granted prorated annual leave commensurate with the regular hours worked as compared to the standard thirty-five hour work week. In general, no more than 2 weeks annual leave may be carried over from one fiscal year to the next.
